RELEASE OF LIABILITY: I am the parent or legal guardian of the student(s) I am registering, am over the age of 18, and have the authority to sign this agreement.  I understand that participation in dance related activities involves a certain degree of risk.  I have carefully considered the risk involved and give my consent for the student(s) to participate in such activities.  I agree to abide by all rules and behavior guidelines set forth now or in the future by Duet Dance Academy, to not engage in any reckless or unsafe conduct while on the premises or at any dance related activity, and to ensure that the student(s) do the same.  I also agree to report any injuries, accidents, or unsafe conditions to Duet Dance Academy as soon as possible.  I release Duet Dance Academy, its owners, managers, agents, employees, contractors, and the owners of the property from any and all liability, claims, demands, and causes of action, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the student(s) and/or the undersigned, while in or upon the premises of Duet Dance Academy, in route to or from any of said premises, or at any other location where a dance related event is held, and I will indemnify, save, and hold harmless, each of the above named parties from any litigation expenses, attorney fees, loss, liability, damage, or costs which may be incurred as a result of a claim.  I understand and agree that any consent, waiver, or indemnification provided under this paragraph shall automatically extend to any family members or friends I or my child may bring to Duet Dance Academy or to any event coordinated or managed thereby, regardless of location.  I understand that appropriate physical contact is required during the instruction of dance and related activities, and I give permission for instructors to make appropriate physical contact with me or my child for such instruction. Duet Dance Academy assumes no responsibility for lost, misplaced, or stolen articles. 

PHOTO/VIDEO RELEASE:  I give permission for Duet Dance Academy to take photographs and/or videos of DDA classes, rehearsals, events, and/or performances including dancers of DDA in costumes of me or my child while participating in DDA activities for promotional, advertising, and social media. Names of students will not be used or disclosed. I understand that I am giving permission to use these photographs and/or videos for publishing use in printed or electronic format and that no financial compensation will be received for this use.
