Please fill out the following form to give permission for your child to exit Duet Dance Acacemy wihtout a staff member physically releasing them to you or your designated pick up person.
Tell us what class(es) you dancer is enrolled in.
This form only applies for students ages 8 to 11 enrolled in our Junior or Elementary age classes. By checking this box, I confirm that my child is 8 to 11 years old.*
Please check each statement below, verifying that you have read and agree to each statement.
I give permission for my child to exit Duet Dance Academy without a staff member physically releasing them to me or my designated pick-up person.*
I understand:
1. My dancer will exit to the designated pickup location (front sidewalk) directly outside the studio.*
2. My dancer will not walk through the parking lot searching for my car.*
3. If I am not parked directly in front of the building, I must walk to the front sidewalk to meet them.*
4. If my dancer does not immediately see me, they will return inside.*
5. Duet staff will not escort dancers to vehicles unless medically necessary.*
6. It is my responsibility to be on time for pickup.*
7. I may revoke this permission at any time in writing by emailing admin@duetdancega.com *
I understand that once my dancer exits the studio, Duet Dance Academy is no longer responsible for supervision.*

By signing below, I authorize Duet Dance Academy to allow my child listed on this form to exit the studio at class and/or rehearsal end time without a Duet staff member releasing them to me or providing supervision outside of the studio. I agree to Independent Exit Policy as detailed in this form.
